Being that your wedding day is one of, if not the most, important day in your life, we took all the steps necessary to make planning for your reception evening as easy as possible. Please look over this form, print it out if you wish and review the questions with your Wedding Consultant(s) and those involved in your decision making process. Once you have decided on how your wish your event to be orchestrated, please return to this page and simply fill in the blanks. Once your have completed this form, and reviewed it, print this for your records and then click the SUBMIT button at the bottom of the form. All your answers will be submitted to our wedding coordinator via email.
